Case Management Program

Members who need housing work with case managers to create and work towards goals. Members use TGP programs and community supports. 

Different objectives include: 

  • Meeting basic needs (shelter, food, personal hygiene, and clothing)

  • Finding and getting employment and benefits

  • Education and training programs

  • Getting stable housing

  • Ordering phones and setting up email

  • Regular transportation

  • Connecting with different supports.

    • Physical health 
    • Mental health 
    • Substance misuse 
    • Crisis planning 
    • Community supports
